The ACS Relay For Life Fundraising Club was established in 1996 as a way to recognize Relay for Life teams that do exceptional fundraising.

GUIDELINES:
* Each team consists of 8-15 registered members. All registration fees must be paid by 6:00 pm on May 18, 2007.
* Registration forms and waivers
must be turned in by 6:00 pm on May 18, 2007.
* All monies to count for the club
must be turned in by 12 noon on May 19, 2007.


LEVELS:

Purple Club - $50,000 or more
Team receives national recognition
Each team member receives a “#1 Teams Nationwide” lapel pin
Each team member receives a special gift
Two team representatives will be invited to the Pennsylvania Division Annual Meeting and Awards Dinner in the Fall
Team is recognized in the PA Division Annual Report
Team is recognized in the Team Fundraising Club book and in Connections, the PA Division Newsletter
Team will receive verbal recognition at the event
Team name and members names will be listed on the Lebanon County Relay For Life web site
Team name and members names will be listed in the wrap up newsletter

Diamond Club - $20,000 - $49,999
Each team member receives a “Diamond Club” lapel pin
Each team member receives a special gift
Two team representatives will be invited to the Pennsylvania Division Annual Meeting and Awards Dinner in the Fall
Team is recognized in the PA Division Annual Report

Platinum Club - $10,000 - $19,999
Each team member receives a “Platinum Club” lapel pin

Gold Club - $7,500 - $9,999
Each team member receives a “Gold Club” lapel pin
Team is recognized in the Team Fundraising Club book
Team will receive verbal recognition at the event
Team name and members names will be listed on the Lebanon County Relay For Life web site
Team name and members names will be listed in the wrap up newsletter

Silver Club - $5,000 - $7,499
Each team member receives a “Silver Club” lapel pin
Team is recognized in the Team Fundraising Club book
Team will receive verbal recognition at the event
Team name will be listed on the Lebanon County Relay For Life web site
Team name will be listed in the wrap up newsletter

Bronze Club - $2,500 - $4,999
Each team member receives a “Bronze Club” lapel pin
Team is recognized in the Team Fundraising Club book
Team will receive verbal recognition at the event
Team name will be listed on the Lebanon County Relay For Life web site
Team name will be listed in the wrap up newsletter
The decision to become the Captain of a Relay For Life team will bring you, more than anything else, great satisfaction, and the
rewarding feeling that you are doing something to enhance the future of Cancer patients, and expand the treatments, services and
options available to them. The work may be hard at times, and the hours may be long, but the benefits are always what keeps us going.

During your tenure as a team captain, you may find that there are many questions. Some like, "Who should I ask to have fundraisers",
"How do I keep my team motivated", or even something as easy as, "How do I get started?" You may find what you're looking for here
within the pages of this site, but if not, please feel free to contact
Laura Donmoyer, Team Mentoring & Support Chair. Your American
Cancer Society staff members are also available to you for any questions, or encouragement you may need. We pride ourselves at being
as helpful as we can, to make your job as team captain as easy & enjoyable as possible! You can contact any of us anytime you have a
question, and we will respond to you as
soon as we can, with as much detailed information as we can.

Also... we will be hosting a Team Captain University on January 21, 2006, which will be a very informal but informative training session for
you, the team captain! We'll also be holding quite a few Relay-wide "open team" fundraisers throughout the 2006 season. Keep your eyes
